* File of Life - This is linked to a PDF. A form full of information about your medical conditions, allergies, medications you are taking
* POLST Form (Physician Orders for Life-Sustaining Treatment) - this is a bright pink form for your refrigerator that says you do not want to be resuscitated if EMT's are called. This must be signed by your doctor and is used if you have a life-threatening illness. If you do not have this form, they are required by law to try to revive you.
* Emergency Contacts - Who should be called? Who knows your passwords and has access to your records? Who knows your wishes? Who should be alerted to care for your pets?
What do you want your family to know? And how do you communicate this?

He has written the wills and Powers of attorney for both Medical and Financial for a number of people in the area. He's also reasonably priced and everyone I know of has liked working with him. Joel Sacks (who was my attorney until he retired) recommended him to me originally, and respects him.
